What is Gout?

Gout is an arthritic condition that produces excruciating pain, swelling, and inflammation in a single joint (although more than one may be affected). It occurs when there is an accumulation of uric acid in the body, leading to the formation of sharp, needle-like crystals in the joints. It usually affects your big toe, but it can also affect other joints in your foot, ankles, knees, wrists, and hands. The discomfort normally begins in the middle of the night while you’re sleeping and gradually worsens over a few hours. The symptoms occur due to a build-up of uric acid in the blood and this causes uric acid crystals to deposit in the joints.

What are the causes of Gout?

The following are typical causes:

Hyperuricemia:

The primary cause of gout is hyperuricemia, a condition in which there is an excess of uric acid in the blood. Uric acid is a waste product formed when the body breaks down purines, substances found in certain foods and drinks.

Diet:

High-purine foods, such as red meat, organ meats, shellfish, and certain fish (like anchovies, sardines, and mackerel), can contribute to elevated uric acid levels.
Alcohol, particularly beer and spirits, can also increase uric acid production.

Genetics:

A family history of gout can increase your risk, suggesting a genetic predisposition to the condition.

Obesity:

Being overweight can increase uric acid production and decrease the body’s ability to eliminate it.

Medical Conditions:

Conditions like high blood pressure, diabetes, kidney disease, and metabolic syndrome can increase the risk of developing gout.

Medications:

Certain medications, such as diuretics (water pills), aspirin, and some drugs used to treat high blood pressure, can raise uric acid levels.

Dehydration:

Not drinking enough water can lead to higher concentrations of uric acid in the blood.

What are the symptoms of Gout?

The following are typical signs and symptoms:

Intense Joint Pain:

The pain usually starts suddenly, often at night, and is typically severe. It most commonly affects the big toe but can occur in other joints.

Redness and Swelling:

The affected joint becomes red, swollen, and warm to the touch.

Limited Range of Motion:

As gout progresses, it may become difficult to move the affected joint.

Lingering Discomfort:

After the severe pain subsides, some joint discomfort may last from a few days to a few weeks.

Tophi:

In chronic or untreated gout, uric acid crystals can form lumps called tophi under the skin around the joints, leading to visible, often painful, nodules.

Is there treatment available for Gout?

The following is treatment to reduce pain and swelling:

Medication e.g. non-steroidal anti inflammatory drugs or colchicine; should be taken as soon as possible

Ice pack/frozen peas on joint for up to 20mins at a time

Keep hydrated

Rest the limb and try to raise it.

Gout is often treated in a variety of methods. To begin, medicine can be given to relieve the pain, and then lifestyle and dietary modifications, as well as possible long-term treatment, can be made to help prevent it from reoccurring.
